Tried mkfs.ocfs2 and mount.ocfs2 from
  http://oss.oracle.com/projects/ocfs2-tools/dist/files/RedHat/RHEL4/i386/1.2.4-1/ocfs2-tools-1.2.4-1.i386.rpm

Same result... mkfs apparently runs fine, but mount fails as before:
ocfs2_dlm: Nodes in domain ("985990656B5044CD922F89FD97A101A1"): 2
(7501,1):ocfs2_find_slot:255 ERROR: no free slots available!
(7501,1):ocfs2_mount_volume:1090 ERROR: status = -22
ocfs2: Unmounting device (8,65) on (node 2)

Executables from
  http://oss.oracle.com/projects/ocfs2-tools/dist/files/RedHat/RHEL5/i386/1.2.6-1/ocfs2-tools-1.2.6-1.el5.i386.rpm
won't run on my system - they get a floating point exception right
away... probably a library conflict of some sort.
