[Ocfs2-devel] DLM bug fixes and cleanups

These patches are against the mainline kernel and have been tested with
the head (2.6.25-rc3+). Once pushed into mainline, they will need to
be pulled into both 1.4 and 1.2 trees.

Except the last one, all other patches have been reviewed by Joel.

The patches have been updated with the comments provided.

The last patch in the series fixes bugzilla#919.

The remaining patches that add features will be mailed again for review
once these bugfixes have been pushed into mainline.
