This has all Eugene's fixes, plus everything I said I'd do in the
replies to Eugene's reviews, plus fixes for usdt args, plus a bunch of
cleanups inspired by Eugene's reviews.

Several tests are expected to FAIL because of a bug I haven't yet
tracked down: test/unittest/pid/tst.{args1,ret1,ret2}.d,
test/unittest/usdt/tst.entryreturn.sh.

test/unittest/operators/tst.str_comparison-basic.d is also failing, but
I believe this to be an unrelated bug to be fixed elsewhere.
